Required Documentation for AHIG Program
Before submitting your application, collect the documents listed below. Accurate documents speed review and improve your chances for funding.
AHIG Membership Program Eligibility Certificate (Required)
All applicants must include the AHIG Membership Program Eligibility Certificate with their submission. This certificate confirms you passed the eligibility check and are authorized to apply for membership grants.
How to obtain the Certificate: Complete the Eligibility Check Form. After successful verification you will be issued the Eligibility Certificate which must be uploaded with your application.
Personal Identification (Individuals)
- Government-Issued Photo ID (Driver's License, Passport, or State ID) — clear, unexpired.
- Proof of Social Security Number (SSN Card or tax document). Black out sensitive financial details if needed.
- Proof of Address (utility bill, lease, or government correspondence within 60 days).
Business Documentation (For Business Members)
- Articles of Incorporation / Certificate of Formation
- IRS EIN Confirmation Letter
- Business License / Registration Certificate
- Operating Agreement or Bylaws (if applicable)
Nonprofit Documentation (For Nonprofit Members)
- IRS 501(c)(3) Determination Letter
- Articles of Incorporation
- Bylaws / Governance Documents
- Most recent annual report or audited financials
Project Proposal & Financials
- Project Proposal — goals, activities, metrics, timeline.
- Budget Breakdown — clear, line-item budget showing how funds will be used.
- Supporting Financial Documents — recent bank statements, profit & loss, or financial projections (if applicable).
Optional Supporting Materials
- Letters of support or partnership agreements
- Photos, program case studies, or evaluation reports
- Resumes or bios of key project staff
File Requirements & Security
- Accepted formats: PDF, JPG, PNG
- Max file size: 10 MB per file
- Privacy: All uploads are encrypted and accessible only to authorized AHIG staff.
- Naming suggestion: Use filenames like
LastName_DocumentType.pdffor quick review.
